Gear lever come loose...

Had the Elise Parts short throw gear lever kit fitted a couple of months back by Ali at SLS. Driving tonight and it just popped right out in my hand?!

I've taken the centre console off to have a look and took a few pics as well. Can't figure out how it is that it should stay in place. Any help on this would be much appreciated.

Image taken from underneath, showing the hard rubber cup that the bottom of the lever sits into


is something missing / broken?

just read it, appears that the grub screw has made a bid for freedom.

obviously wasn't "thread locked in place"
